If NPRK feels my pet needs medical care, I the undersigned give permission to NPRK to take my animal to a vet. I understand I am responsible for my pet and all costs it may incur. Furthermore, if I leave mt pet 96 hours past the "date out" without contacting and paying NPRK for all costs to date, my pet will be considered abandoned and it shall be removed from NPRK at my expense. (May be given away, submitted to the Humane Society, etc.) I agree that NPRK nor any of it's employees are not liable for loss, death, illness (even due to bacteria or virus), injury or accidental breeding.
